On your first visit, you will be asked to
fill out the Patient History Form. Below
is a link to it.
If
you like, you can download this form, print, fill it out
at home, and bring it with you on your appointment date.
Filling
out the form ahead of time will save you time in the waiting
room.

You

Form. If you like, you can download the forms, fill them out at home, and bring them with you to your appointment. The forms are in PDF format and you will need Adobe Reader to view the file. Alternatively, you may wait until you get to the center to complete
the paperwork.
At your appointment, you will be given additional questionnaires
regarding pain, activities of daily living, etc. to fill out. You will then
meet with one of our doctors, who will review your problems with you and answer all of your questions, and then recommend the optimal treatment for you.
Some of our patients have complicated problems of longstanding
duration. In these cases we recommend that you forward us
your medical file in advance. These records should include results
of diagnostic testing, information from your most recent medical examination, a list of all of your medications, and any other documents that would help doctors appreciate the complex nature and
extent of your problem. All original records will be
returned to you.
Our treatment policy demands that we see a noticeable improvement in your condition in the first three visits. The improvement can include: a decrease in pain, an improvement in
sleep, an increase in activities of daily living, or an improvement in
objective measurements such as thermal imaging, EMG, laser Doppler, somatosensory testing,
etc. If we cannot see any improvement after three visits, we will discontinue treatment. This does not mean that you should expect to be symptom-free after three visits, but you should see at least some improvement by then.
